When someone relocates they often seek information about physicians, specialists, hospitals and other types of health care, such as extended care. Is there a resource in Brooklyn Park for health care information?








The 4NOW program offers:
• Annual exams
• Testing and treatment of sexually transmitted infections (STIs) identified during an annual exam
• Birth control
• Family planning education
You can enroll in 4NOW if you’re:
• between ages 15 - 50
• a Minnesota resident
• a U.S. citizen (or non-citizen eligible to participate in federally funded programs)
• not pregnant
• not enrolled in Medical Assistance (MA), MinnesotaCare, General Assistance Medical Care, or another state-funded program
• earning no more than the weekly income limits

Sign up for 4NOW at the Planned Parenthood nearest you! You’ll need to bring two things:
· Proof of income - either a current pay stub or a letter from your employer verifying your weekly wages.
· Proof of citizenship - either a copy of your passport, birth certificate, a photo ID, or immigration documents.
